New Patient Registration

The doctors welcome new patients who live within our practice area.

Medical treatment is available from the date of registration.  Please contact reception for further information.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, Adult Questionnaire and  Child Questionnaire which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Before we can register a new patient we need to see a driving licence or passport and a recent utility bill. If you were resident overseas but are now residing in this country we need to see supporting documentation from an employer etc.

The NHS in England is introducing the Summary Care Record, which will be used in emergency care.  The record will contain information about any medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had to ensure those caring for you have enough information to treat you safely.  For more information regarding the Summary Care Record a detailed letter can be downloaded here: Summary Care Record If you do not wish to have a Summary Care Record an Opt Out form can be downloaded here: Opt Out Form  Please bring the form into the surgery for your notes to be marked accordingly.
