New Patient Registration

The doctors welcome new patients who live within our practice area, and our reception staff will be happy to guide you through the registration process. 

This involves completing the below 3 forms (please note there are different patient questionnaires for adults and children) and presenting them to our reception with your ID - either a driving licence or passport and a recent utility bill (dated within the last 3 months), and a birth certificate for all children under 16. If you were resident overseas but are now residing in this country we need to see supporting documentation from an employer etc.

The NHS in England has introduced the Summary Care Record, which will be used in emergency care.  The record will contain information about any medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had to ensure those caring for you have enough information to treat you safely.  For more information regarding the Summary Care Record a detailed letter can be downloaded here: Summary Care Record. If you do not wish to have a Summary Care Record please complete the below Opt Out form and bring this into the surgery with the other 2 forms for your notes to be marked accordingly.
